

Youth After Care Support Service
(YACSS)
Case support option for young people wanting to achieve sustainable long term behavioural change. A new programme offered by the Link Youth Health Service funded by the Department of Health and Human Services.
What does YACSS offer?
· YACSS provides a case manager whose primary focus is to develop and implement individual relapse prevention care plans. This service offers support for clients who require ongoing post withdrawal support.
· Linking the young person into supported referrals and improved social networks.
· A flexible service that can adapt to the varying needs of the client from low level contact through to regular intensive case support.
· YACSS provides seamless transition into other services offered by The Link including access to GP and psychologists
Who Is Eligible?
A young person is eligible for YACSS if they are:
· under 25 years of age
· Residing within southern Tasmania
· Engaged with a withdrawal service (e.g. Detox unit, Bridge Rehabilitation Service) and have made a genuine commitment to withdraw
· Have a moderate to high level of motivation to change.
How do I refer someone to YACSS?
Simply contact The Link Youth Health Service on 6231 2927 to discuss the needs of the client and determine eligibility
